Oral Health Assessment Program
The Oral Health Assessment Program requires that by May 31 of each year, students in kindergarten (or first grade if it is their first year in public school) submit proof of an oral health assessment by a licensed dentist or other licensed or registered dental professional. The law states that schools must send notification of the assessment requirements to parents or legal guardians. Districts collect Oral Health Assessment Forms from schools, aggregate the data and submit the information to the county office by December 31 of each year (The Annual Oral Health Assessment Data District Reporting Form). Please note: The data submitted by December 31, reflects the previous academic school year.
