Oral Health Assessment

The Oral Health Assessment Program requires that by May 31 of each year, students in kindergarten (or first grade if it is their first year in public school) submit proof of an oral health assessment by a licensed dentist or other licensed or registered dental professional. The law states that schools must send notification of the assessment requirements to parents or legal guardians. Districts collect Oral Health Assessment Forms from schools, aggregate the data and submit the information on the System for California Oral Health Reporting System (SCOHR) https://www.ab1433.org/home/ by July 1 of each year. Please note: The data submitted by July 1 reflects the previous academic school year. If your school, district or COE needs access, please contact the San Joaquin County Office of Education (SJCOE), CEDR (CodeStack) department. Information is located at the bottom of the SCOHR Homepage. If your account has already been set up, enter your username and password into the fields and click the Login button.
